ForwardHealth is the name for the health care and nutritional assistance benefit programs administered by the Wisconsin Department of Health Services (DHS). These programs include BadgerCare Plus, Wisconsin Medicaid, and other programs that provide medical and mental health insurance coverage for those with limited income. You can apply for these programs online, by phone, by mail, or in person.
Milwaukee Enrollment Services (MilES) manages enrollment in ForwardHealth programs for Milwaukee County. An individual may call the MilES Call Center or walk into any location and get assistance. A MilES worker can provide assistance to all individuals, regardless of the location or the method of contact (phone, online, or in-person at one of three locations listed below).
